*Transport outside the 35mile radius: $1.50 per loaded mile additional
*Transport are available out of state: Price will be quoted on a specific case basis
Terms of Services:
The services provided by Atlanta Mortuary Transport LLC will include an AMT representative that arrives in a timely matter at the location of the decedent, to be taken into AMT’s care, and then transported to the designated funeral home in an appropriate AMT transport vehicle. If needed, a second AMT representative will be on scene to assist in the removal of the decedent. .
The payment procedure for these services provided by AMT are as follows:
Payment must be remitted 48 hours after receiving AMT’s invoice and transport of the decedent to final destination. These fees are Non-refundable. (Except by the specific direction of the undersigned. Who may terminate this service agreement. Upon termination in writing, a full refund will be provided by AMT.)
Acknowledgment and Understanding:
The undersigned or appointed family member, acknowledges the right to arrange for AMT transportation services and are bonded by the terms of each individual agreement.
